Our team coordinates your patient's specialty care, communicates the treatment plan and, when surgery is needed, works with you to coordinate post-operative co-management when appropriate.

From Referral to Co-Management
1. You Refer
When your patient needs specialty ophthalmic care, send a referral to Eye Care Specialists through our online referral portal or by fax.

2. We Coordinate
Our team reviews the referral and helps connect your patient with the appropriate ophthalmologist for their needs.

3. We Evaluate
Your patient meets with an Eye Care Specialists ophthalmologist for a specialized evaluation. Our team discusses the diagnosis, treatment options and recommended next steps with the patient.

4. We Keep You Connected
Communication is an important part of the referral process. We keep your practice informed of your patient's diagnosis, treatment plan and next steps so you remain connected to their care.

5. We Treat
When surgery or specialized treatment is recommended, our team coordinates the patient's care and guides them through the process.

6. We Co-Manage
When appropriate, post-surgical care can be co-managed with the referring optometrist.
Our team coordinates the transition of care and communicates the information needed for continued post-operative management, allowing your patient to return to the provider they already know and trust while remaining connected to our specialty team.

Post-Surgical Co-Management
Co-management allows Eye Care Specialists and referring optometrists to work together throughout a patient's surgical care.

Our ophthalmologists provide the surgical expertise, while referring optometrists may participate in appropriate post-operative care. Through communication and coordination between our practices, patients experience continuity of care before, during and after surgery.
